Move Buff Bar Wow
This looks pretty simple. From what i can see the player and target frames are standard WoW interface but moved using the Move Anything addon.Then it looks like bartender for the bars.And the target health bars over to enemy head looks like Tidy Plates.Then of course Recount on the right for DPS meters.I personally really like Move anything because it's super light weight and doesn't affect your FPS as opposed to downloading new UI's. I've used Pitbull UI and x-perl and they just bogged my system down. This pic is very similiar to how I have my UI set up but I have standard WoW bars.Edit: wrong link, spelling.
Could try which should allow you to move and resize the buff bars. Buff bar addons have the problem these days that they either lack customisation or the ability to do.

In my UI I move the buffs towards the bottom of the screen where I have moved most of my UI elements. I also resize them.
